Client Waiver To Get Tattooed

The form below is for clients getting tattooed at the Kamloops Tattoo Show.

All clients must be 18 years of age, with 2 pieces of ID.

* IMPORTANT INFORMATION *

You will need a new waiver for each tattoo.

This waiver is considered signed consent to receive tattoo services at the Sandman Centre during the Kamloops Tattoo Show 2025 and the artists there in.

Before getting tattooed you must obtain a sticker by our waiver staff, this sticker is your proof that you have been approved to be tattooed.

No sticker, no tattoo.

By submitting this application and selling merchandise at the Kamloops Tattoo Show, you confirm that you have the legal right and/or proper authorization to sell all materials, products, or services offered at your booth. The Kamloops Tattoo Show is not responsible for verifying ownership or authorization of any merchandise sold by vendors.

Applications will not be processed unless deposit is received. Please note that all payments made after the application is accepted are non-refundable.

Once you submit this form, our team will send you a contract for signature along with an official invoice. Upon receiving the invoice, a 50% deposit is due at signing to secure your spot. The remaining balance must be paid in full by May 1st.

Be cautious of fraudulent requests. We will never ask for deposits or payments outside of our official invoices.
